NW Momentum Health Partners Accountable Care Organization (NWMHP) received a 94.25% quality rating score and achieved $1.3M in shared savings for the 2019 reporting year in the Next Generation Accountable Care Organization Model.
The Next Generation ACO Model was launched in 2016 by the Centers for Medicare & Medicaid Services. The Model allows NWMHP to set predictable financial targets, enables providers and beneficiaries’ greater opportunities to coordinate care, and aims to attain the highest quality standards of care.
The goal of the federal Next Generation ACO Model is to test whether strong financial incentives for ACOs, coupled with tools to support better patient engagement and care management, can improve health outcomes and lower expenditures for Original Medicare fee-for-service (FFS) beneficiaries.
