As COVID-19 rapidly evolves, NW Momentum Health Partners (NWMHP) – a Physicians of Southwest Washington (PSW) ACO – has increased its focus on supporting high-risk beneficiaries that may fall victim to the social isolation circumstances in Washington state and Idaho.
ACOs are accountable for managing Medicare beneficiaries, a population particularly vulnerable to COVID-19. The elderly and those with underlying chronic conditions such as diabetes are most at-risk for serious side effects, according to the Centers for Disease Control and Prevention.
As a result, NWMHP is actively reaching out to identified patients to provide additional resources such as; Remote Patient Monitoring, care management services including meal delivery, transportation services and coordination of pet food to ease the financial burden and stress associated with self-quarantine.
For its provider network, PSW is coordinating supplies for its independent providers, hosting a COVID-19 informational webinar and has launched a discussion platform designed to keep physicians connected and informed.
